Summarize all of your information in a typed report following this guide. This section of the project is to be written in APA format (1” margins, Times New Roman, 12pt font). Add other pertinent comments. Start with a title page and an introduction. End the written assessment and analysis with your reference page.

Introduction:
I’m a dental hygiene student, I’m studying full-time. In addition, on the weekend I also work as a nail technician. Therefore, I’m very busy with my schedule, I try to balance myself between school, work, friends, and family. It’s extremely difficult for me to have the right diet, which has enough nutrients for my body. For the meals, I only want to cook or buy food that is quick and simple, this could not have sufficient nutrients…
Body of Paper:
- Social history: Discuss reasons why you choose the foods you consume. Use classroom notes and the textbook to get other ideas for food choices. Examples:
- Exercise? I work out a few days a week
- How often eat away from home? I eat out with friends and family during weekend
- Who does cooking/shopping? My mom does the cooking and shopping for food
- Who lives at home? My parents and my sister
- Number meal/snacks per day? I eat 2 meals a day, lunch and dinner. I usually eat snack at night before bed time while watching movie or listening to music
- Working? Number of hours/week? Regular hours? I work 40 hours a week

- Medical history: List all pertinent information as it appears on your medical history form from the clinic, including family history of disease, any surgeries, medications, smoking, supplements, and OTC and prescription drugs. Determine your BMI. Comment on findings. Include relationship of medication, alcohol, or smoking to your nutritional status. Which nutrients are affected? What are some suggestions for improvement?
I’m in good health, I do not take any medications. However, I do take a multivitamin daily. I drink alcohol and smoke vape.
- Dental history: List all pertinent information from your dental/oral history form from the clinic, including:
- Oral hygiene status (i.e., amount of plaque): Generalized healthy and localized gingivitis on the molar teeth.
- Periodontal problems—includes a description of gingival condition: pink, firm, knife-edge and slightly pigmented
- Orthodontics: I have not.
- Number of restorations: 4 amalgams on the molar teeth.
- Does your dental health affect nutritional intake? Does your nutritional intake have an impact on your oral health? No
- Special dietary considerations (e.g., cultural, lactose intolerant, low fat, low calorie, allergies, intolerances, vegetarian, religious, or do you follow no special guidelines): Include foods you like and foods you dislike.
Vietnamese culture uses quite a lot of oil when they stir-fried food, I’m lactose intolerant, and I don’t really like eating vegetables. I like to eat Vietnamese beef noodle soup which is called Pho. I don’t like to eat cheeses food since it makes me fat.

- Conclusions and expected success of the program: What have you started to change, or do you anticipate changing your diet while in school? List at least three specific and measurable goals/objectives you plan to make this year. What did you do well? Summarize all of your comments on improving our dietary intake. Create a realistic menu for 1 day.
