Instructions

1. Review the rubric to make sure you understand the criteria for earning your grade.
2. After reviewing your various learning experiences, input from your practicum supervisor and other relevant parties, seminar assignments, revised Self-Assessment, Learning Contract, completed Clinical Observation Feedback Tool, and other materials, write a paper in which you appraise areas in which you have grown and areas where you have room for improvement.
   1. Identify a minimum of two areas of strength and two areas of growth needed.
   2. Explain your strengths and growth areas based on feedback or experiences in your practicum and give specific examples.
   3. Also, identify at least two action steps you will take going forward or additional items you will add to your learning contract to address your growth areas in the next seminar.
3. Write a minimum two-page paper, following appropriate APA Style conventions, such as cover page, double spacing, one-inch margins, formal spelling/grammar, references where appropriate, and Times New Roman font size 12. If you do reference or cite materials, please create a reference page, which is not included in the page count.
