Referral Letter

In many settings, rehabilitation counselors will not be able to complete these assessments
themselves – either because of time constraints or professional credentials. Therefore, the ability
to formulate sound referrals and to secure the necessary evaluative data is important. On the
other hand, their case documentation might be essential to determinations of eligibility for
services or selection of appropriate services. The purpose of these assignments is to provide
familiarity with the essential elements of good referrals and the corresponding evaluations.
Each student shall select a client type as the basis for each of these assignments (50 points each).

This should represent one of the major diagnostic categories from the DSM-5. This project will
have two parts:

Completion of a referral letter. The referral letter will be from the Rehabilitation Counselor to the Psychologist or Psychiatrist doing the evaluation.

Use proper business letter format.
