This rehabilitation is designed for people who need close daily physician monitoring, 24-hour registered nurses and intensive physical, occupational and/or speech therapy to maximize recovery. We provide comprehensive programs for therapy, and other treatments for neurological disorders, cardiac conditions, and orthopedic and spinal cord injuries This is short-term inpatient rehabilitation for patients recovering from an accident, injury or illness. Programs: Northwell health stern family center for rehabilitation is a 256 bed skilled nursing facility that specializes in short term rehabilitation and extended care services.

Staff 2 nurses Practitioners and 6 medical MD’S staff and 1 medical director .They have 8 units  Nurse practitioners not assigned to a specific floor or patients helping if 1 doctor is out for the day responded to patients complaints reported by the nurses while making rounds .

91 YEARS old male Spanish speaking speaks English too with hx od HTN neuropathy BILATERAL LEGS  LEF ANKLE FRACTURE .Patient admitted to hospital secondary to persistent diarrhea for 4 weeks and was recently started on gabapentin 4 weeks ago prior hospitalization for nerve pain and started having diarrhea .patient had a GI consult patient was + for Ecoli and was on antibiotic cipro completed a few days ago  .Patient was hypotensive and vomited blood during hospitalization .GI consult s/o EGD  showed gastric ulcer with path intestinal metaplasia  .Plavix AND ASA D/C started sulcrafate as prophilaxis .EGD follow up recommended by GI in 8 weeks and  patient also had stones  without hydronephrosis  on ct scan seen by urology no further interventions and patient was sent to Rehab

Patient was seen for anemia h&h 7.2/23.5 the day before was 8.9/28.4 .Went to examine the patient she was c/o of feeling fatigued  with intermittent SOB that started over the weekend.denied abdominal pain but on exam noted suprapubic tenderness .

Ask nursing staff  to obtain PVR and UA g\for urine culture .Later in the afternoon nurse reported patient had dark loose stool.FOBT ordered

Next day

Ask per staff nurse to see patient Patient  noted to have SOB ,Diffuse abdominal pain worsened since last night ,no appetie feeling weak and dizzy fatigued  noted to have bloody loose stool with clots and bloody emesis  and patient was sent to hospital   for further evaluation and to be transfused

Medication coreg 25mg

Sulcrafate tablet 1 gm 4xday

Protonix twice daily
