Each state has a Department of Aging and a website associated with the department. Typically on the website, you will find a Guide to Services. The guide is the resource you will be using to create your weekly fact sheets for a total of 4 fact sheets. After completing your weekly fact sheet you will submit the fact sheet as an assignment using the assignment link above, additionally, you will post the fact sheet to the weekly fact sheet discussion board. This discussion board will be for sharing of the fact sheets only. You are expected to post each week your new fact sheet but you are not responsible for responding to fellow classmates’ threads. The discussion board will become resources of information for all classmates to review. The grade for your weekly participation in the discussion board fact sheet submission will be a portion of the grade earned by your weekly assignment submission.
Locate your state’s Department of Aging website and Guide to services
Review the table of contents
Each week you will select 2 topics you believe are associated with the objectives for the week
Once you have selected the week’s topics of interest, your research begins
Visit that area of the guide
Using the fact sheet template provided, you will begin to thoroughly complete the sheet
To accurately complete the fact sheet needed information, You are required to contact a minimum of four (4) of the resource areas identified for each topic
Once you have completed the fact sheet you will submit the fact sheet using the assignment link
You will also share your fact sheet with your classmates using the Weekly Guide to Services discussion board link
Your fact sheet assignment submission and discussion board post will be graded using the fact sheet rubric
